Include setup, result and revision evidence.
Front tab slipped at the second card gives a later reader more to work with than did not work. Include enough setup information to repeat the observation and explain why the proposed change addresses it.
Write enough setup information for another reader to repeat the test. A result without its conditions can be difficult to interpret.

Describe the setup. Record the observation. Link a proposed change.
'Front tab slipped at the second card' is more useful than 'failed'.
Could someone repeat the test? Use short labelled fields.
