Activity Feedback Sheet Builder

Print an optional activity feedback sheet with editable prompts and drawing boxes or writing lines.

Use an activity name, not a child’s name. Keep each prompt short and focused on the activity.

Check the activity and prompts, then create the sheet.

Ask about an activity children have tried

Allow around 3–5 minutes. Provide pencils or broad drawing tools. Read the prompts aloud and let children point, draw, write, use a scribe or pass. The sheet asks whether they would choose the activity again and leaves space for two ideas. There are no correct answers and no name field.

For younger children, use drawing boxes and a concrete prompt such as “Draw something you would change.” For older children, use writing lines and ask for a reason or a practical change. Keep your questions about materials, instructions, time or the activity itself; do not ask for private family information.

Use the responses to make a change

Explain how suggestions will be considered and tell the group what you can change. A “no” or a skipped response is allowed. Do not treat a preference as an assessment of ability, wellbeing or a learning outcome. Collected paper responses stay with your program; this tool does not record them.

Keep and print the template

Choose A4 or US Letter. Print or Save as PDF at 100% scale with browser headers and footers off; check the preview before making copies. Save settings to keep the activity name, prompts, paper size and response layout on your device. Nothing is uploaded. The settings file contains the template, not children’s feedback.
